Your Role As the Implementation Manager for Evergreen Connected Care (ECC), you own the day-to-day execution of practice onboarding and the ongoing operating relationships with our partner practices. Connected Care is delivered as a white-labeled extension of the practice and its providers; you are the person who makes each launch happen on time and keeps the relationship running well afterward. You run the standard onboarding sequence at each practice - kickoff, implementation and breakout meetings, final all-hands, and the controlled outreach start - and you hold the timeline. You build and refine the meeting materials, facilitate sessions alongside the Program and Clinical Director or on your own, track every open item to closure, and make sure the right people know what is expected of them and by when. This is an execution and coordination role. The operating model already exists; your job is to run it consistently as the program scales and keep work from falling through the cracks. This position is fully remote, with the possibility for occasional travel to on-site meetings with practices. Role Responsibilities Some responsibilities may vary based on the practices you support, but this role's primary duties include the following: • Practice Onboarding and Implementation • Run the ECC onboarding sequence end-to-end for each new practice, from kickoff through first outreach and the post-launch ramp review. • Hold the working timeline across multiple concurrent implementations and escalate slippage early. • Coordinate the parallel implementation and clinical workstreams so the two tracks stay in sync, including patient data intake and validation, forms and materials, outreach wording, after-hours handling, billing pathway, and system access. • Confirm and maintain the practice contact map: leadership, project champion, clinical lead, data/EHR, and billing. • Meetings and Materials • Build agendas and prepare, refine, and version the meeting materials and decks used across kickoff, breakout, and all-hands sessions. • Facilitate meetings with the Program Director or independently and drive each session to decisions and named owners. • Capture and circulate follow-ups after every meeting, and confirm the practice completes its post-meeting forms and deliverables. • Prepare recurring internal and practice-facing status updates and materials for leadership review. • Partner Practice Relationships • Serve as a primary day-to-day point of contact for partner practice leadership, administration, clinical leads, billing, and EHR/data contacts. • Manage expectations on timing, data requirements, and practice-side responsibilities, and keep communication timely and clear. • Surface practice-side risks and blockers to the Program Director before they affect launch dates or patient outreach. • Tracking, Follow-Through and Process • Maintain a single source of truth for deadlines, commitments, and open items across all practices, and drive them to closure rather than simply reporting them. • Partner with the Clinical Program Director on coordinator staffing readiness, training timing, and launch pacing. • Coordinate with internal Finance, Legal and Compliance, Data, People Operations, and Market Operations counterparts to unblock practice-facing work. • Manage vendor and platform coordination required for launch and steady state, including the technology partner's implementation and account teams. • Identify where the process breaks repeatedly and propose durable fixes - checklists, templates, trackers - so each launch is cleaner than the last. • Other duties consistent with this role, as assigned. Required Qualifications • 5+ years of experience in implementation, program or project management, client success, or practice/provider operations, ideally in healthcare services, value-based care, or healthcare technology. • Demonstrated experience running multi-stakeholder implementations to a defined timeline with external customers or partners. • Excellent written and verbal communication skills, including the ability to build a clean deck and facilitate meetings of clinicians and executives. • Strong organizational discipline and follow-through; comfortable operating with autonomy and ambiguity. • Advanced skills with MS Office Suite of products including Outlook, Teams, SharePoint, Excel, and PowerPoint. • Willingness to travel occasionally for practice kickoffs and on-site sessions. • Able to work effectively in a primarily remote environment: • Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Compensation The pay range for this role is $100,000-$110,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors. Your Role As a Sr. Analyst, Reporting & Analytics with Evergreen Nephrology, you will report to the Manager of Reporting & Analytics and collaborate with the broader analytics, technology, and operations teams to develop data-based solutions and tools that drive company performance and patient outcomes. Your reporting (e.g., Power BI) and analytical skills (SQL) will help develop new reports and tools, and your communication skills and business acumen will drive collaborative efforts in gathering requirements and feedback, providing analytic insight to our stakeholders, and drive improvements to our operating and clinical models. Your Role As a Telephonic Pharmacy Care Coordinator with Evergreen, you play a critical patient-facing and operational role in our pharmacy program. This is a 6-month contract with the opportunity to convert to full-time employment. This program connects patients with a specialized mail-order pharmacy that provides pre-sorted, multi-dose medication packaging (pill packs) along with proactive refill management and periodic care team check-ins, helping ensure patients stay on track with complex medication schedules. Working fully remotely by phone, you are often the first voice patients hear from the pharmacy care team — helping establish trust, explain the value of the pharmacy service, and guide patients through enrollment. You work with patients from our partner practices to engage them, capture consent, and set up timely access to medications and pharmacist-led clinical services. You'll explain how the service works — multi-dose pill packs organized by date/time, home delivery and automatic refill coordination so patients never run out, and scheduled check-in calls to review adherence and address new prescriptions or dose changes. When a patient agrees to participate, you act directly in the electronic health record — updating the patient's preferred pharmacy, triggering electronic prescriptions, and documenting consent or decline based on the patient's decision. Your primary focus is high-volume outbound outreach to patients who do not have an office visit, or who cannot be supported onsite, along with inbound support as needed. This role is focused on telephonic patient engagement, consent and enrollment, EHR documentation, and care coordination for patients living with chronic kidney disease and other complex chronic conditions. Success depends on your ability to connect with patients quickly over the phone, communicate value clearly, and remove the barriers that keep patients from staying consistent with their medications. Role Responsibilities Some responsibilities may vary based on specific patient programs, but this role's primary duties include the following: • Conduct high-volume outbound telephone outreach to patients to introduce Evergreen's pharmacy service, establish rapport and trust, and guide patients through enrollment. • Deliver clear, standardized messaging over the phone — using approved scripts and talking points to drive consistent, informed enrollment decisions and manage patient questions and objections. • Obtain and document patient consent by phone, and capture the information needed to enroll the patient. • Make the necessary changes in the EHR based on the patient's decisions — updating the patient's preferred pharmacy, triggering electronic prescriptions (eRxs) to the pharmacy partner, and accurately documenting consent or decline in the patient's chart. • Manage outreach queues and follow-up cadences, completing multiple contact attempts per patient and prioritizing outreach based on urgency and targets. • Accurately document all call activity, outreach attempts, accepts/declines, and coordination activities in Evergreen's clinical and operational systems, supporting accurate enrollment and success-rate tracking. • Meet call center performance expectations, including call quality, productivity, average speed of answer, and enrollment/conversion targets. • Uphold patient privacy and confidentiality standards in accordance with HIPAA and organizational policies. • Other duties consistent with this role, as assigned. Your Role As a Licensed Practical Nurse (LPN) with Evergreen Nephrology, you are responsible for conducting monthly clinical check-ins, identifying and addressing clinical concerns, reinforcing care plans, escalating issues appropriately, and ensuring clinical documentation accuracy across both the EHR and partner technology platform. This position requires clinical judgment, licensed expertise, and the ability to support patients between office visits while collaborating closely with Evergreen’s providers, care teams, and leadership. LPNs are essential to delivering a safe, compliant, clinically credible Connected Care Program. The program uses SMS as the first-line channel for patient engagement, with phone outreach used when appropriate. While this position is fully remote, you must be able to work from 8:30am – 5pm in the pacific time zone. Role Responsibilities Some responsibilities may vary based on specific patient programs, but this role's primary duties include the following: • Patient Engagement & Outreach • Perform monthly CCM touchpoints for assigned patient panels. • Build strong, ongoing relationships with patients and caregivers through regular outreach. • Review patient chart data (diagnoses, kidney disease history, medications, labs, imaging, treatment cycles) before calls. • Explain CCM participation and reinforce program benefits. • Clinical Assessments & Judgment • Conduct structured symptom assessments (pain, fatigue, neuropathy, SOB, nausea, etc.). • Evaluate changes in condition using licensed clinical judgment. • Assess medication adherence issues, side effects, or confusion about regimen. • Identify clinical red flags requiring escalation (e.g., worsening symptoms, new SOB, uncontrolled pain, concerning vitals). • Escalation, Coordination & Provider Communication • Escalate urgent issues based on defined triage pathways (same-business-day or immediate). • Collaborate with the provider team on symptom changes, new concerns, or medication-related issues. • Route EHR messages with clinically relevant documentation. • Assist in coordinating care: scheduling follow-up, arranging labs, ensuring nephrology appointments are kept, and reducing missed care. • Care Plan Reinforcement & Patient Education • Reinforce provider-developed CKD and chronic disease care plans. • Provide clinical explanation of symptom management instructions (within LPN scope). • Support adherence to medications, lifestyle changes, and follow-up instructions. • Identify social, behavioral, or access barriers and determine appropriate interventions or referrals. • Documentation, EHR Updates & Compliance • Document all patient interactions clearly in both the EHR and CCM patient management platform. • Track and log qualifying CCM time accurately for billing compliance. • Ensure documentation meets regulatory, internal QA, and partner expectations. • Maintain high call quality and adherence to scripting and workflow pathways. • Quality, Performance & Continuous Improvement • Maintain or exceed: • ≥80% monthly engagement • ≥90% billable conversion • High QA/documentation accuracy scores • Participate in peer reviews, QA audits, coaching sessions, and continuous education. • Identify recurring patient needs and communicate trends to the Program Manager. • Other duties consistent with this role, as assigned.
