We're launching a new Longevity Program focused on healthy aging — comprehensive geriatric assessment, chronic disease management, cognitive health, and weight management including GLP-1 therapy. The role You'll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth. This includes comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and management of chronic conditions. You'll work alongside our CMO, a board-certified geriatrician, with full training and clinical support provided. What you'll do • Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk • Provide medical follow-up visits for chronic disease management and ongoing care • Perform cognitive screening and staging; escalate for further assessment where indicated • Prescribe and manage GLP-1 therapy for appropriate patients • Conduct advance care planning conversations • Document in our EHR to support accurate Medicare billing Requirements Requirements • Active, unrestricted NP license in Florida • Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging • Active DEA registration • Experience with Medicare documentation and E/M coding • Comfort with telehealth and EHR documentation • Reliable high-speed internet and a private, HIPAA-compliant workspace Nice to have • Experience with comprehensive geriatric assessment • Familiarity with cognitive screening instruments (Mini-Cog, MoCA) • GLP-1 prescribing experience • Multi-state licensure Benefits Why Total Life • 100% remote • Flexible scheduling • Patients come to you — no panel building or marketing required • Clinical training and ongoing support from our geriatrician CMO • Meaningful work with an underserved population
